Online Athletic Forms
The District uses an electronic process for completing required athletics and activities paperwork. The required forms are available via the IC Parent Portal and can be completed at any time for the upcoming school year. Forms must be completed before a student’s first day of practice/participation. Parents/guardians must complete the required forms for each participating high school student. Once a parent/guardian submits a form, the student will have access to sign the form via their IC Student Portal.

- Hardcopies of the WIAA Physical Form and WIAA Alt Year Permit Card will continue to be required; these forms will NOT appear in IC as available to complete electronically and can be downloaded below.
- WIAA Physical Forms and WIAA Alt Year Permit Cards can be provided to your child’s coach or athletic director.
